The intent of the Kentucky HIV/AIDS Care Coordinator Program (KHCCP) is to facilitate the provision of quality care and services to HIV-infected individuals and their families in a timely, consistent manner of continued care. The program provides care coordinators in the Cumberland Valley region to assess needs and provide services. The Cumberland Valley Regional Care Coordinator Program serves the following counties: Adair, Bell, Breathitt, Casey, Clay, Clinton, Cumberland, Floyd, Green, Harlan, Jackson, Johnson, Knott, Knox, Laurel, Lee, Leslie, Letcher, Magoffin, Martin, McCreary, Owsley, Perry, Pike, Pulaski, Rockcastle, Russell, Taylor, Wayne, Whitley
& Wolfe.
KHCCP also is an umbrella program for other client assistance programs such as the Kentucky Health Insurance Continuation Program, Outpatient Health Care and Support Services and state support service programs. KHCCP receives federal funds and is a service-funding source of last resort. Financial assistance is not guaranteed. Funding is limited and services may be terminated without cause. Services in each region may vary depending on funding. Continuation of all programs is contingent upon availability of state and federal funding. For questions please feel free to contact us.
