1) Local Health Nurse II – Home Health (Bell, Clay, Harlan, Jackson, & Rockcastle Counties)
The Cumberland Valley District Health Department is accepting applications for a Full Time Local Health Nurse II – Home Health on an ongoing basis for the counties of Bell, Clay, Harlan, Jackson & Rockcastle counties.
General Duties include: This position serves under limited direction of the Nurse Supervisor. Responsibilities for this position include, but are not limited to; Conduct comprehensive nursing assessments for clients that range from simple to extremely complex. Determine program eligibility, needed services and payer source. Develop an individualized plan of care in consultation with the client’s physician and other appropriate agency personnel. Complete review of medications, assess pain management and need for health care equipment/technology. Provide intermittent skilled nursing services, as ordered by the physician. Obtain specimens, as ordered and submits to State Lab utilizing appropriate Laboratory Services Guidelines. Identify health risks and implements risk reduction strategies for self and others according to agency policy. Conduct or participate in team meetings/case conferences to evaluate client’s response to care, alter the care plan and make referrals to other community agencies. Develop/conduct educational counseling/teaching activities for the individual clients and families. Provide thorough documentation and submit appropriate reports, as required. Provide increasing leadership and supervision. Participate in utilization review for quality of services. May mentor LHN I – HH nurses. Minimum Education, Training or Experience: See special requirements. One (1) year of Registered Nurse (RN) experience.
Substitution for Education, Training or Experience: BSN or Masters Degree in Nursing, Nursing Administration, Nursing Education or Public Health may substitute for the one (1) year experience. Special Requirements: (Age, Licensure, Regulation, Etc.) Must have RN license in Kentucky or compact state.
Starting Salary: $15.79 (75 hours bi-weekly) Grade 19

2) Local Health Nurse II-Public Health (Bell, Clay, Harlan, Jackson & Rockcastle Counties)
General Duties include: This position serves under the direction of the Nurse Supervisor. Responsibilities for this position include, but are not limited to; Perform work with increasing independence and may assume minimal mentoring of LHN I nurses. Provide nursing care and assessment to the client consistent with the Public Health Practice Reference (PHPR), Nursing Scope and Standards of Practice. Develop and implement a plan of care based upon identified health risks. Document services provided to the client. Stay updated with policies, procedures and licensing laws. Maintain confidentiality and privacy according to guidelines. Understand the team process and apply problem solving strategies to accomplish job tasks. Participate in specialized or program specific clinics. Participate in employee orientation and training. Develop and conduct educational counseling and teaching activities for individual clients and families. May serve as a coordinator of a specific program. Minimum Education, Training or Experience: See special requirements. One (1) year of Registered Nurse (RN) experience.

3) Senior Home Health Aide (Bell, Clay, Harlan, Jackson & Rockcastle Counties)
The Cumberland Valley District Health Department is accepting applications for a Full Time Senior Home Health Aide on an ongoing basis for the counties of Bell, Clay, Harlan, Jackson & Rockcastle counties.
General Duties include: This position serves under the basic direction of a Registered Nurse. Responsibilities for this position include, but are not limited to; Provide moderate to difficult Home Health services with increased knowledge, skills and abilities.
Performs appropriate and safe techniques in personal hygiene and grooming that include:
Bed sponge, tub or shower bath/shampoo, sink tub or bed, nail and skin care, oral hygiene. Toileting and disposal, safe transfer techniques and ambulation with or without equipment. Normal range of motion and positioning, adequate nutrition and fluid intake. general homemaking task, respite services. Performs other duties as assigned in classification.
Observation, reporting and documentation of patient status and the care or service furnished. Perform as preceptor during orientation and training of home health aides. Work as lead person in scheduling, coordination, orientation and training of Home Health Aides and participate in quality assurance activities related to aides.
Minimum Education, Training or Experience: High School Diploma or GED. Two (2) years of compensated experience in a home health agency, all or part of which must have been in the previous 24 months prior to employment.
Substitution for Education, Training or Experience: Appropriate aide related experience may substitute for the education requirement on a year for year basis for a maximum of four (4) years. For promotional purposes an individual with two (2) years of experience in the home health agency may be considered for this position.
Special Requirements: An individual employed in this position must maintain continued fulfillment of the required competency evaluation approved by the Cabinet for Health Services.
Starting Salary: $8.90/hr. (75 hours bi-weekly) Grade 10

Experience as an aide in a home health agency, community health agency, nursing facility or hospital, all or part of which must have been in the previous 24 months prior to application for employment.
Special Requirements: To become permanent, an employee must pass a competency evaluation prior to completing the probationary period. An individual employed in this position must maintain continued fulfillment of the required competency evaluation approved by the Cabinet for Health Services.
Starting Salary: $8.10/hr. (75 hours bi-weekly) Grade 8
